Comments:
Renewal study was conducted with the provider and one assistant. Ten children were present the age range of the children was 12 months to three years old. Children were observed during rest time, free play and story time. Six children's records were reviewed , the provider and three house hold members were also reviewed. Inspection time 1:20pm-4:00pm

Violations:
Standard #: 22VAC40-111-770
Description: Based on observation the provider did not have a working weather band radio. Evidence: the provider had a radio however it would not work during the inspection.

Plan of Correction: New radio has been purchased.

Standard #: 22VAC40-111-940-C
Description: Based on observation not all cups of milk were stored in the refrigerator. Evidence: there was a large bowl of plastic cups stored on the floor many of the cups had left over milk from lunch. LI observed at 3:30 pm a toddler take one of the cups out and drink from it.

Plan of Correction: The large plastic bowl has been moved to suggested area out of children's reach.

Standard #: 22VAC40-111-60-A
Description: Based on review of six children's files not all were complete. Evidence: Child A with a start date of 9/7/17 did not have the street address for father's work or the work number for mom.

Plan of Correction: All information has been updated.

Standard #: 22VAC40-111-240-A
Description: Based on observation , the provider did not maintain areas and equipment in the day care area in a safe condition. Evidence: An infant sleeping in a crib had a pacifier attached to a stuffed animal (wubbanub).

Plan of Correction: That child no longer has a wubbanub.

Standard #: 22VAC40-111-320-A
Description: Based on observation not all small electrical appliances were unplugged when not in use. Evidence: A toaster and coffee pot were plugged in and not in use.

Plan of Correction: We will unplug coffee pot when not in use.

Standard #: 22VAC40-111-560-A
Description: Based on observation the room used for sleeping infants was to dark to have sight of the infants. Evidence: caregivers and LI could not see the infants sleeping in cribs due to the room not having widows or a light.

Plan of Correction: Night light has been purchased.
