Comments:
An unannounced monitoring inspection was conducted on April 2, 2018, between the hours of approximately 10:55 a.m. and 2:30 p.m. Thirteen (13) children were enrolled and eight children were in the care of the provider and one assistant during the on site inspection. Five children's records, the provider's record and one caregiver's record were reviewed and/or updated. No medications were administered. Safety equipment (first aid kit, flashlight, radio and fire extinguisher), facility documents and posted information were observed/reviewed. Program observation was completed. Activities observed on this date included: indoor play; diapering, restroom and handwashing procedures; lunch; and naptime. Adult/child interactions, behavior guidance and daily routines/transitions were also observed. The children knew and followed the daily routine and transitions well. Exceptions to compliance with the licensing standards were noted on the violation notice. Discussion on this date included: fingerprinting procedures; posted information and caregiver training. Standard #: 22VAC40-111-60-B
Description: Based on review of five children's records and discussion with the provider on April 2, 2018, the program failed to maintain a record for one child. Evidence: 1. Review of children's records, on this date, established that a record was not on site for child A. 2. The provider stated the record had been returned to the child's parent for updating/completion and had not been returned.

Plan of Correction: If child A returns for care (after today) a complete record will be obtained and on file.

Standard #: 22VAC40-111-170-A
Description: Based on review of one caregiver's record and discussion with the provider, the program failed to obtain a current report of tuberculosis (TB) screening at the time of hire. Evidence: 1. Review of caregiver #1's record, on this date, determined that a TB exam report/screening was not on file. 2. Discussion with the provider and caregiver confirmed that a TB exam/screening was not on file.

Plan of Correction: A TB exam/screening will be obtained.

Standard #: 22VAC40-111-320-G
Description: Based on review of documents and discussion with the provider on April 2, 2018, the program failed to obtain an annual fireplace and associated chimney inspection. Evidence: 1. Documentation determined that the last fireplace/chimney inspection was conducted on January 30, 2017. 2. The provider verified that an updated/annual inspection had not been completed.

Plan of Correction: A fireplace/chimney inspection will be completed.
