Description: Based on record review, children's records were incomplete. Evidence: 5 of 5 children's records were incomplete. 1) Children A and B were missing the insurance policy number 2) Child C was missing the addresses of 2 of 2 emergency contacts. 3) Child D was missing the addresses of 2 of 2 emergency contacts and father's work phone number. 4) Child E was missing the mother's work phone number.
