- Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file. Currently, the Family Day Home as obtained finger print checks for all applicable household members. Discussion that all 5 full size cribs observed on site were manufactured after June 2011.
The following inspection took place between the hours of 10 am through 1:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Chantilly, Virginia. At the start of this announced initial inspection there were a total of 4 children (1 resident child) (9 points) with 1 caregiver. A total of 4 children?s files, 1 staff file, and 5 household member?s files were reviewed. The children were engaged in the following: free play, toy play, games, hand washing, diapering, imaginative play, infant feeding, infant sleeping, lunch, nap time, and other organized activities. Standard #: 22VAC40-111-750-A Description: Based on review and interview, 3 of 4 children's files reviewed indicated that there was neither written parent authorization for the topical skin product of diaper cream to be used on site, nor was there a noting of any known adverse reactions obtained for this year. Evidence - 1. The Provider stated (during an interview conducted on the date of inspection - 04/10/2018) that 1 child out of 4 was potty trained and that the other 3 needed diaper cream applied on an as needed basis. 2. On the date of inspection, there was not a AUTHORIZATION TO APPLY A NON-PRESCRIPTION TOPICAL SKIN PRODUCT form in the following children's files: Child #1, Child #2, and Child #3. Plan of Correction: AUTHORIZATION TO APPLY A NON-PRESCRIPTION TOPICAL SKIN PRODUCT form shall be completed and placed on file for all applicable children.
Standard #: 22VAC40-111-790 Description: Based on observation, emergency numbers were not posted. Evidence - 1. On the date of inspection (04/10/2018), the following phone numbers were not posted: 911, and the number for poison control. 2. The Emergency Preparedness & Response Plan that contains the number of the person to be used in an emergency was posted. Plan of Correction: All applicable numbers shall be posted on the main bulletin board.
Standard #: 22VAC40-111-60-B Description: Based on review, 1 of 4 children's files reviewed indicated that it did not contain all of the required documentation/information. Evidence - 1. On the date of inspection (04/10/2018), the action to take in an emergency situation for the allergy listed of Child #3 was not documented. 2. The proof of Child #3's age and identity was not available for review. The start date for Child #3 was listed as - 08/14/2017. Plan of Correction: All missing documentation/information shall be obtained and placed in the applicable child's file.
Standard #: 22VAC40-111-330-A Description: Based on interview and observation, the Family Day Home (FDH) does not have a working landline telephone service installed. Evidence - 1. The Provider stated (during an interview conducted on the date of inspection) that she had called to obtain this service, but the current phone for this landline was not working. 2. There was not dial tone when the telephone was connected to this landline. Plan of Correction: The phone company shall be contacted to address and fix this issue. In the meantime, all parties has the FDH cellphone number.
Standard #: 22VAC40-111-430-A Description: Based on observation, children under two years of age and children over two years of age who are not developmentally ready to climb or descend stairs without supervision did have access to stairs. Evidence - 1. On the date of inspection (04/10/2018), the back stairs, leading into the Family Day Home (FDH) or leading out and up into the backyard did not have a gate to prevent access by children under two years of age. 2. On the date of inspection, there was one child in care that was under two; another child under two was not present but is also enrolled. Currently, the parents and children enter the day care through this back entrance. Plan of Correction: A gate shall be installed at the top of these back stairs.
