Comments:
A Subsidy Health and Safety inspection was conducted on Monday, April 16, 2018 to review supplemental health and safety requirements. There were six (6) children present under the direct care & supervision of 1 caregiver. Ratios were within licensing requirements. The Provider continues to participate in the USDA Child Care Food Program, receives CACFP training, does not administer medication and does not provide transportation for the children in care. Children and staff/household records were reviewed during the inspection. A Licensing Renewal Inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations of the supplemental subsidy health and safety requirements were found. An exit interview was conducted with the Provider on 4/16/2018. Standard #: 22VAC40-111-100-A
Description: Based on record review, the Licensee failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. EVIDENCE: Record for Child #4 did not include documentation of the child's most recent physical exam. Child #4 was enrolled on 8/21/2017

Plan of Correction: Provider will contact the child's parent to get documentation of the child's most recent physical examination

Standard #: 22VAC40-111-540-A
Description: Based on observations, the Licensee failed to use safety straps when using infant/toddler equipment. EVIDENCE: Two (2) children were observed sitting in 2 separate infant bouncer seats; safety straps were not used.

Plan of Correction: Provider will make sure safety are always used when occupied by a child. Corrected during inspection.
