An unannounced monitoring inspection was conducted on April 30, 2018 between the hours of approximately 10:15 a.m. and 1:30 p.m. Thirteen children were enrolled and ten children were in the care of two caregivers on this date (24 points). Five children's records were reviewed and/or updated. The provider's record, assistant's record and household members' records were updated. No medications were administered, as is the policy of this family day home. Facility documents (attendance form and emergency evacuation and shelter-in-place drill record), posted information (evacuation diagram, menu, emergency telephone numbers, daily schedule, license and licensing inspection documentation) and safety equipment (first aid kit, flashlight, radio and fire extinguisher) were observed/reviewed. Program observation was completed. Activities observed on this date included: indoor play; television; lunch; naptime; and infant care. Adult/child interactions, behavior guidance and daily routines/transitions were also observed. On this date, lunch included: chicken nuggets, pears, corn and milk. Substantial compliance with the licensing standards was maintained. One exception to compliance was noted on the accompanying violation notice. Description: Based on review of caregiver and household members records and discussion with caregiver #1 on April 30, 2018, the program failed to obtain a current tuberculosis (TB) screening every two years from the date of the first screening. Evide4nce: 1. Review/update of records for caregivers #1 and #2 and household member #3, on this date, determined that updated TB exam reports/screenings had not been obtained. Dates of initial screenings included caregiver #1 - 4/14/2016, caregiver #2 - 4/12/2016 and household member #3 - 4/13/2016. 2. Caregiver #1 confirmed the lack of updated TB exam reports/screenings.

Plan of Correction: Updated TB screenings/exam reports will be obtained for all adult household members and caregivers.
