All caregivers need to have completed fingerprinting no later than September 30, 2018. Any caregiver that interviews after 1/23/2018 needs to have documentation of fingerprinting before being hired and starting work. If a caregiver has lived in another state(s) in the past 5 years, a search of the central registry (CPS check) must be conducted for each of those states. Information on how to obtain this information can be found on our website at dss.virginia.gov.

Three children's files and one assistant file were reviewed. There are no medications on site - Medication Administration Training is required to administer medication to day care children. Standard #: 22VAC40-111-460-D Description: Based on provider interview, the provider failed to have documentation of the latest rabies vaccination for household pets. Evidence: the provider could not locate the documentation of the latest rabies vaccination for three household cats. Plan of Correction: Provider stated the cats are current in the rabies vaccination and will contact the vet to get documentation if it can not be located.
Standard #: 22VAC40-111-570-A Description: Based on observation and record review, the provider failed to ensure that each caregiver does not exceed 16 points using the point system based on children's ages at any given time. Evidence: on 4/30/2018, there were twelve children present, ranging in age from 3 months to 4 years, totalling 34 points. There were two caregivers present, but three were required. Plan of Correction: Provider miscounted the points of the children that attend on Mondays. Provider will have an additional staff person come in to help on Mondays and any other day the points exceed 32 points.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of one caregiver file, the provider failed to have documentation of a search of the central registry finding (CPS check) within 30 days of employment.
