An unannounced monitoring inspection was conducted on May 21, 2018 from approximately 10:00 am to 12:10 pm. Provider and two assistants were present with seven children at the onset of inspection. Children were observed playing freely and eating lunch. Five children's records and two caregiver records were reviewed. No medication is being administered and transportation is not provided.
Standard #: 22VAC40-111-60-B Description: Based on review, provider did not ensure 2 of 5 children's records have documentation of required information. Evidence: On May 21, 2018, the following was found during record review: 1. The record for child #2 does not have documentation of father's employment address, father's employment phone number and medical insurance plan number. 2. The record for child #3 does not have documentation of father's employment address and father's employment phone number. Plan of Correction: Provider will get with the parents this evening to get missing information.
Standard #: 22VAC40-111-180-A Description: Based on review, 2 of 2 caregiver records did not have documentation of updated TB tests or screening within the past two years. Evidence: On May 21, 2018, the records for caregiver #1 and caregiver #2 had a most recent TB test completed January 18, 2016 which is more than two years old. Plan of Correction: TB tests will be done.
Standard #: 22VAC40-111-210-A Description: Based on review, 2 of 2 caregivers did not have documentation of completing at least 16 hours of training annually. Evidence: On May 21, 2018, the following was found during record review: 1. Caregiver #1 completed 12 hours of annual training in 2017. 2. Caregiver #2 completed 10 hours of annual training in 2017. Plan of Correction: All staff will have 16 hours of training this year.
