Comments:
An unannounced monitoring inspection was completed today. The physical plant, 2 caregiver/household member files, 4 children's files, caregiver training, children's activities, emergency procedures, drills and supplies were inspected between 2:40-4:00. There were 4 children, totaling 9 points, under the supervision of the Provider. Today, the children played with toys in the sand, enjoyed free play outside, had reading time, listened to music and rested during naptime. The children bring their lunches to the FDH each day that they are in care. The Provider is not certified in MAT (medication administration training) and there are no children with any medication needs. Description: Based on file review, the Provider did not practice emergency evacuation drills each month children are in care. Evidence 1. The most recent evacuation drill was completed on 12/20/17

Plan of Correction: These will be completed and documented.

Standard #: 22VAC40-111-60-B
Description: Based on file review, 4 out of 4 children's records do not have all required information. Evidence 1. Children #3 and #4 do not have addresses for the 2 emergency contacts in case the parents cannot be reached. 2. Children #1-4 do not have the addressed for their parent's work.

Plan of Correction: The Provider will ask the parents to complete this.

Standard #: 22VAC40-111-70-A
Description: Based on file review, 1 out of 4 children's records do not have all required written information given to review from the Provider. Evidence 1. Child #1 does not have a medication policy on file.

Plan of Correction: The Provider will give a copy of this to the parent to sign.

Description: Based on interview, the Provider does not have a current certification in CPR. Evidence 1. The most recent CPR certification for the Provider expired 4/16/18. This has not been recertified.
