Technical Assistance:
The provider has obtained the field print code but not fingerprinting. LI (licensing inspector) reviewed with the Provider the required forms that are required to be in each child's record.

An unannounced monitoring inspection was conducted today between the hours of 10:15 am through 11:45. There were a total of 3 children (10 points) with 1 caregiver. A total of 3 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play, snacks and coloring. Areas of non-compliance are identified in the violation notice. Discussed the importance of correcting all violations while on a Conditional license, several violations were repeats from the prior inspection. Standard #: 22VAC40-111-800-A
Description: Based on record review, the provider does not have a completed emergency preparedness and response plan. Evidence: The provider has a Fairfax County emergency plan that does not contain all the state requirements.

Plan of Correction: I completed the form but it is with my husband. I will take the form and file it.

Standard #: 22VAC40-111-960-E
Description: Based on observation, the provider has not labeled with child's full name and dated each infant bottle. Evidence: There are 2 children that are taking infant bottles. Neither of the child's bottles were labeled with the child's name or dated.

Plan of Correction: I will label and date them everyday.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children records do not contain all the required information. Evidence: Child #1 record does not have emergency contact address, medical insurance policy number, proof of birth information completed and Information for parents form completed.. Child #2 does not have a complete name, address and phone number for emergency contact, proof of birth completed, results of physical examination signed by a physician and Information for Parents form completed. Child #3 does not have a second address for emergency contact, medical insurance policy number, proof of birth completed, and Information for Parents form completed.

Plan of Correction: I will have the parents complete the missing information.

Standard #: 22VAC40-111-70-A
Description: Based on record review, the parents have not been provided the required information before the child's first day of attendance. Evidence: Child #1, #2 and #3 have not been provided information about medication administration policy, liability insurance and emergency preparedness plan procedures.

Plan of Correction: I will provide the parents with the information and file the forms in the child's record.
