05/01/2017 Arrived 11:25am Departed 1:15pm An unannounced monitoring inspection was conducted today. Thirteen children were in care for a total of 22 points with one caregiver. The children were eating lunch, playing with toys and resting. Eight children's files and two staff files were reviewed. Thank you for your cooperation. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on the review of eight children's files and an interview with the provider, the facility failed to obtain a physical health exam within 30 days of enrollment for each enrolled child. Evidence: 1. There is no physical health record on file for child #3 (enrolled 3-14-17) or child #9 (4-6-18).

Plan of Correction: The provider reported that she will obtain immunization records for all children.

Standard #: 22VAC40-111-40-A
Description: Based on observation the provider failed to insure that the licensed capacity was not exceeded. Evidence: 1. There are 13 children in care.

Plan of Correction: The provider will insure that the capacity is not exceed again. A parent brought two children who were not expected to be in care today.

Standard #: 22VAC40-111-60-A
Description: Based on the review of eight children's files and an interview with the provider, the facility failed to have a file for each child in care. Evidence: 1. There is no file available for review for child #9 who began care on 4-6-18.

Plan of Correction: The provider reported that the file was returned to the parents to obtain more information. The parent confirmed this and is bringing the file today.

Standard #: 22VAC40-111-90-A
Description: Based on the review of eight children's files and an interview with the provider, the facility failed to obtain an immunization record for each enrolled child before the child attends. Evidence: 1. There is no immunization record on file for child #3 (enrolled 3-14-17) or child #9 (4-6-18).

Plan of Correction: The provider reported that she will obtain immunization records for all children.

Description: Based on observation the provider failed to insure that the points requirement (16 per caregiver) was not exceeded. Evidence: 1. There are 13 children in care for a total of 22 points with one caregiver.

Plan of Correction: The provider will insure that the capacity is not exceed again. A parent brought two children who were not expected to be in care today.
