Technical Assistance:
Technical assistance was provided regarding the correct authorization for medication forms.

Comments:
An unannounced monitoring inspection was conducted today from 9:30am-11:15am. There were 5 children (16 points) supervised by 2 direct care givers.. The home, staff records, children?s records, household member?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing, and eating snack. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-111-700-A
Description: Based on review of two medications, the center did not have written authorization to administer medication to a child in care. Evidence: Provider does not have the correct form of authorization to administer medication to child #1 & 2.

Plan of Correction: I will contact the parents and get the correct form.

Standard #: 22VAC40-111-100-A
Description: Based on review of five children?s records, the provider did not maintain complete children?s records. Evidence: Child #1 did not have documentation of a physical examination within 30 days after first day of attendance.

Plan of Correction: I called mom and will get a copy.
