Comments:
An unannounced monitoring inspection was conducted today, June 4, 2018 to determine the provider's compliance with family day home standards. The inspection was initiated at 10:30 am and concluded at 1:00 pm. Upon the inspector's arrival, nine children were in care with two caregivers for a value of 24 points. Shortly after an additional caregiver arrived. Children were observed singing and dancing to a music video. Songs identified body parts and children pointed to them as they sang along. The three infants were sleeping at the time of the inspector's arrival. Later children had time outside to play with small trikes and toys. Upon returning inside children had bathroom breaks, diapering was conducted and hand washing. Lunch time was also observed. Today's lunch consisted of pasta, tomato sauce, mozzarella cheese, applesauce, mixed vegetables and milk. All areas used for child care were inspected along with a walk through of the entire home. The family day home has age appropriate materials and supplies. The provider had the following required information posted: The license, last inspection notice, emergency numbers, allergies, menu, and attendance sign in and out sheets. The first aid kits and non-medical emergency supplies were inspected and found complete. The inspector interacted with caregivers and children in care.

Three caregivers records and nine children's records were reviewed.
The provider has current MAT. Medications, allergy action plans were reviewed.
The most recent evacuation drill was documented on 05/21/2018. The two required shelter-in-place drills for 2017 were completed on 06/29/2017 and 12/11/2018. A lock down drill was completed on 06/29/2017.

Transportation is not provided.
The emergency preparedness plan was reviewed and current.

Violations:
Standard #: 22VAC40-111-120-B
Description: Based on review of caregiver records, the provider did not obtain all required information for each assistants' and substitute providers' records. Evidence: The record of caregiver #3, hired 02/08/2018, did not have documentation of two references that were checked before employment.

Plan of Correction: Per the provider: I will be sure to document references in file.

Standard #: 22VAC40-111-170-A
Description: Based on review of caregiver records, the provider did not obtain documentation of a current tuberculosis screening for each caregiver at the time of hire. Evidence: Caregiver #3, hired 02/08/2018, had documentation of a tuberculosis screening dated 04/03/2018, exceeding the requirement to obtain at the time of hire.

Plan of Correction: Per the provider: I was unaware TB tests need to be completed at hire. I will make sure all new hires have TB done before hire date.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of caregiver records, the provider did not obtain a central registry finding within 30 days of employment or volunteer service. Evidence: Staff #3, hired 02/08/2018, had a central registry finding dated 04/05/2018, exceeding the 30 days from hire allowance to obtain.

Plan of Correction: Per the provider: I will send the request for the central registry off at hire to ensure it is returned within the 30 days from hire.
