54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted on 4/25/18 from 11:20 am to 1:00 pm and on 6/5/18 from 1:00 pm until 2:00 pm. At the time of the inspection on 4/25/18 five children were present with the provider and one household member and on 6/5/18 four children were in care with the provider. The sample size consisted of five children's records and the provider's record and one household member's record. The provider and children were observed during free play, structured art and educational activities, hand washing, lunch, nap, transitions, and behavioral guidance. The violations from the previous inspection were reviewed for current compliance. One repeat violation was found. The violation is documented on the violation report. Standard #: 22VAC40-111-100-A Description: Based on record review, the provider failed to obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance for one out of five children. Evidence: The record for child 4 contained documentation of a physical dated 5/7/18. The start date documented in the record for child 4 was 2/26/18. Plan of Correction: In the future the provider will ensure each child's record is complete before the child's first date of attendance.
