A Subsidy Health and Safety inspection was conducted on 4/25/18 from 11:20 am to 1:00 pm and on 6/5/18 from 1:00 pm until 2:00 pm to review supplemental health and safety requirements. At the time of the inspection on 4/25/18 five children were present with the provider and one household member and on 6/5/18 four children were in care with the provider. The sample size consisted of five children's records and the provider's record and one household member's record. The provider and children were observed during free play, structured art and educational activities, hand washing, lunch, nap, transitions, and behavioral guidance. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on interview, the provider failed to ensure lockdown procedures were practiced at least annually. Evidence: The documentation of emergency drills did not contain documentation of a lockdown drill being completed. The provider stated she has never completed a lockdown drill.

Plan of Correction: The provider is no longer going to provide subsidy care.
