Comments:
An unannounced monitoring inspection was conducted today from approximately 9:30am-11:40am. There were 12 children in care one of which belonged to the provider ( 30 points) directly supervised by 2 caregivers. A sample of 5 children?s records, 2 caregiver records and 3 household member records were reviewed. The children were observed during bathroom procedures, eating snack, coloring, building, engaged with caregivers and playing. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov.

Violations:
Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider did not review with the parent the emergency contact information to ensure the information is correct. Evidence: The record of Child B was last reviewed 3/22/2017.

Plan of Correction: The parent will update the file today.

Standard #: 22VAC40-111-800-A
Description: Based on interview with provider, the family day home did not have a written emergency preparedness and response plan that. Evidence: The provider stated that she was unable to locate the emergency preparedness and response plan.

Plan of Correction: I will fill out the plan and it will be available for the next visit.

Standard #: 22VAC40-111-830-A
Description: Based on record review, emergency evacuation procedures were not practiced at least monthly. Evidence: The emergency procedures were not practiced for the month of June 2018.

Plan of Correction: I will have a drill this month.

Standard #: 22VAC40-111-960-E
Description: Based on observation, bottles were not labeled with the child's full name and date. Evidence: The bottle of Child A was not labeled with the date.

Plan of Correction: I will add the date today.

Standard #: 22VAC40-111-210-A
Description: Based on record review and interview with provider, the caregivers did not complete ate least 16 hours of relevant training annually. Evidence: The Provider stated that she and her assistant completed 16 hours of training for the 2017 year. The documentation of the completed training was unable to be located during the inspection.

Plan of Correction: I will send a copy of the training once I find the record.
