Comments:
An unannounced monitoring inspection was conducted on 07/09/2018 from 11:05am - 1:00pm. There were eight children in care with two caregivers. The ages of the children counted for a total of 14 points. Children were observed playing with toys in the play room and participating in a music and movement activity with the assistant. Lunch as additionally observed. The sample size included two caregiver and four children's records. No medication is being administered at this time. The violations are listed on the violation notice and were reviewed with the provider at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-111-780-B
Description: Based on record review and interview, the provider failed to ensure that annually the parent shall review the emergency contact information to ensure the information is correct, and obtain the parent's signed acknowledgment of the review. Evidence: 1. The record for child 2 was last reviewed by the parent on 11/21/2016. a. The provider confirmed this as the most recent update and review by the parent.

Plan of Correction: The provider stated: Each parent will be asked to review and update, as needed, the enrollment record today at pick-up.

Standard #: 22VAC40-111-60-A
Description: Based on interview, the provider failed to ensure that each child has an up-to-date record. Evidence: 1. There is no enrollment record on file for child 1 who was in care during the inspection. a. The provider stated she did not have enrollment forms for child 1.

Plan of Correction: The provider stated: This child is a drop in for today and tomorrow only. The parent was provided with all of the enrollment forms however, she did not return today. The parent will be asked to return all completed enrollment forms tomorrow morning at drop off.

Standard #: 22VAC40-111-90-A
Description: Based on interview, the provider failed to ensure that before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized . Evidence: 1. Documentation of immunization information was not on file for child 1 who was in care during the inspection. a. The provider confirmed that immunization documentation was not on hand for child 1.

Plan of Correction: The provider stated: Child 1 is in care for today and tomorrow only. The parent will be asked to provide immunization documentation tomorrow at drop off.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider failed to ensure that outside areas of the family day home shall be maintained in a safe condition. Evidence: 1. One fence slat was loose revealing two exposed screws at the bottom which is accessible to children. a. These two screws were rusty and had sharp points.

Plan of Correction: The provider stated: A new fence slat will be put in place. The old one with the exposed screws will be discarded. This will be done today.
