Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, July 25, 2018. The on-site inspection was conducted from 10:50 a.m. to 1:45 p.m. On this date, there were seven children in the care of the provider and assistant (15 points). There were a total of 12 children enrolled in the program. There were five children's records reviewed and the records of three caregivers and one adult household member were reviewed for updates. The provider, the assistant and two children were interviewed. There were no medications being administered at the time of the inspection, which is the policy of the provider. The exception to compliance was noted on the violation notice and the risk rating for the violation was noted on the supplemental page. Lunch was observed to meet USDA guidelines and consisted of chicken, mashed potatoes, lima beans, rolls, and milk. Some items reviewed and observed included story time, posted information, refrigerator and freezer temperatures, first aid kit and emergency supplies, choice time, bathroom and hand washing procedures, infant feeding, diaper changing procedures, lunch, and nap time. Standard #: 22VAC40-111-830-A
Description: Based on review of records and discussion with the provider, the provider failed to practice emergency evacuation procedures each month. Evidence: 1. The record of evacuation drills showed that an evacuation drill had not been conducted every month. The date of the last documented evacuation drill was 03/05/2018. 2. The provider reported that drills were completed in April and May but she failed to document them. She reported that she had not conducted one in June.

Plan of Correction: The provider will conduct two evacuation drills in July, one of which will be a make-up drill for June. From now on, she will make sure to record every drill.
