Comments:
An unannounced Subsidy Health and Safety inspection was attempted on September 5, 2018. The provider was not available at that time. A second inspection was conducted on October 12, 2018. The provider was present. There were no children in care at the time of the inspection. Documentation for the provider and emergency preparedness was reviewed. Emergency procedure drills had been documented. It was noted that transportation and medication administration are not provided. Inspection began: 11:20 Inspection ended: 12:40

Standard #: 63.2-1721-B-2
Description: Based on a review of documentation and information given by the provider, the letter of eligibility based on the finger print criminal record search that was to be completed by September 30, 2018 was not on file. Evidence: The provider had not obtained the letter of eligibility from the finger print criminal record search. (The Virginia State Police Criminal Record Clearance was on file.)

Plan of Correction: The clearance will be initiated within 10 days. Documentation will be sent to the licensing inspector.
