Technical Assistance:
Discussed with provider standard 22VAC 40-111-500B. The highest climbing rung or platform on outdoor climbing equipment or top of a slide shall not exceed six feet for school age children, and four feet for preschool children.

Comments:
An unannounced monitoring inspection was conducted today from 10:20am-12:45pm. There were 4 children (10 points) directly supervised by 2 staff. The home, 1 staff record, 4 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, finger painting, completing worksheets, playing outdoors, and eating snack. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified on the Violation Notice. Violations:
Standard #: 22VAC40-111-60-A
Description: Based on record review, the provider did not maintain complete children's records. Evidence: Child#3's record is missing the child's medical insurance policy number. Child#4's record is missing the mother's work address.

Plan of Correction: We will request the information today.

Standard #: 22VAC40-111-70-A
Description: Based on review of children's records, the provider does not have documentation that parents were provided in writing required information before the child's first day of attendance. Evidence: Child#1's record is missing the information for parents form, child#2 is missing the authorization for emergency medical care, and provisions of the family day home's emergency preparedness and response plan.

Plan of Correction: We will have the form ready at pick up for parents to sign.

Standard #: 22VAC40-111-80-A
Description: Based on record review, the provider did not obtain a proof of age and identity for each child within seven business days of the child's first day of attendance. Evidence: The records of child#2, #3 and #4 did not contain documentation that proof of age and identification was verified within seven business days of enrollment. Child#2's first day of attendance was documented as 4/23/2018, child#3's first day of attendance was documented as 5/16/2016, child#4's first day of attendance was documented as 2/5/2018

Plan of Correction: We will ask parents to bring the required proof of age by end of today.

Standard #: 22VAC40-111-180-A
Description: Based on review of staff records, the provider did not obtain updated tuberculosis (TB) tests or screening for each household member every two years. Evidence: Household member#1's file contains documentation of a TB test/screening that expired on 1/8/2018. Household member#2's file contains documentation of a TB test/screening that expired on 1/8/2018.

Plan of Correction: We will go tomorrow to get the TB screening/test done.

Standard #: 22VAC40-111-240-A
Description: Based on record review, ares of the home were not maintained in a safe a operable condition. Evidence: In the infant room a crib (not in use) was observed missing a screw on the upper left corner. In the main programming area a small black chair was observed with a tear about 2 inches wide in the fabric exposing the inside foam.

Plan of Correction: We will replace the chair, and we will remove the crib because is not in use.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on record review and interview with the provider, the provider did not obtain a central registry report within 30 days for a 14 year-old residing in the home. Evidence: Household member#3's file does not contain a central registry report.

Plan of Correction: We will fill the form out and send it to the agency.
