Technical Assistance:
Licensing inspector discussed Pre-Service training with provider.

Comments:
A Subsidy Health and Safety inspection was conducted on 08/02/2018 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from approximately 11am-145pm. There were 8 children in care (21 points) directly supervised by 2 caregivers. A sample of 4 children?s records, 2 caregiver's records and 2 household member's records were reviewed. The children were observed playing outside, eating lunch, watching television, napping, playing with toys, during diapering procedures and washing hands. One new caregiver has been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on records reviewed, the vendor did not maintain a record of daily attendance that documented the arrival and departure of each child in care. Evidence: The attendance was not documented for the following days: July 23 2018-July 27 2018 and August 1 2018.

Plan of Correction: I will update my attendance today.

Standard #: VENDHOM-000-012-A
Description: Based on record review and interview with provider, the provider has not completed the Virginia Pre-Service Training. Evidence: The provider stated that she has not yet completed the Virginia Pre-Service training.

Plan of Correction: I will complete the training by the next visit.
