54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced renewal inspection was conducted beginning at 1:00 p.m. and ending at 2:30 p.m. with the provider. There were five children in care ranging in age from twenty months to eight years of age for a total of nine points. Children were observed coloring and having free play. Childcare areas and supplies were observed. Five children's records, one provider record and five household member records were reviewed. Emergency drills were reviewed. Staff training was reviewed. The provider has completed 14 hours of training during the license year and has until 8/31/2018 to complete an additional 2 hours of training. The provider is not Medication Administration Training (MAT) certified and cannot administer medications to children in care. Description: Based on review of documentation, the provider failed to obtain a sworn statement for household member 18 years of age. Evidence: Household member #4 turned 18 on 5/8/2018 and did not have a sworn statement in the file.

Plan of Correction: The sworn statement will be completed today.

Standard #: 63.2-1721.1-B-2
Description: Based on review of documentation, the provider failed to submit fingerprinting for household member 18 years of age. Evidence: Household member #4 turned 18 on 5/8/2018 and did not have fingerprinting results in the file.

Plan of Correction: An appointment for fingerprinting will be made as soon as possible.
