54.1-3408 Provider must be MAT certified to administer prescription medication.
An announced initial inspection was conducted today from approximately 10am-1:35pm. There were 3 children in care (7 points) directly supervised by 1 caregiver with the assistance of the daughter of the provider. A sample of 3 children?s records, 1 caregiver's record and 3 household member's records were reviewed. The children were observed watching television, playing electronics and engaged with the daughter of the provider. Areas of non-compliance are identified in this report. Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were incomplete. Evidence: 3 of 3 children's records were missing required information. Children A-C were missing the father,s residential information, father's employment information, and 1 of 2 required emergency contacts. Plan of Correction: Parents will update files today.
Standard #: 22VAC40-111-320-A Description: Based on observation, small electrical appliances were not unplugged. Evidence, The coffee machine located in the kitchen was plugged in and not being used by the provider. Plan of Correction: Corrected during inspection.
Standard #: 22VAC40-191-40-C-1-a Description: Based on record review and interview with provider, household member's of the family day home did not have a central registry search on file. Evidence: Household member A did not have a complete central registry search on file. Plan of Correction: It was submitted two months ago and I have not received the results. I will follow up with the office of background investigation to get the results.
Standard #: 63.2-1721.1-B-2 Description: Based on record review, household members have not completed fingerprinting. Evidence: Results of completed fingerprinting procedures were not on file for the provider, Household Member A, Household Member B and Household Member C. Plan of Correction: Fingerprinting was completed for all household members and Me. I will follow up with the office of background investigation to get my fingerprinting results. Since I was permitted Fairfax County they did not send me the results of the fingerprinting procedure that I completed in February 2018.
