Comments:
An unannounced renewal inspection was conducted beginning at 9:20 and ending at 10:30. There were 4 children in care which included two resident children. The provider was in appropriate ratio. Reviewed provider and family files - complete Reviewed 2 daycare children's records - see violation notice. Menu was reviewed. Attendence sheet was complete. Emergency drills are complete Family records are complete. Large supply of age appropriate play things for children of all ages. Standard #: 22VAC40-111-60-B
Description: Based on a review of the records of the two children in care, it was determined that the records were not complete. Evidence: Child A and Chld B - had one of two required emergency contacts. Child B did not have a physical/well baby check.

Plan of Correction: Provider will notify the parent today.
