54.1-3408 Provider must be MAT certified to administer prescription medication.
- Technical Assistance:
Provider needs to go online and down load renewal application and submit. License expires on October 3, 2018. Provider has an attendance record that lists arrival and departure for each child, each day to ensure points are met the and does not need to also keep the FDH attendance sheet.
An unannounced renewal was conducted today at 125 PM- 235 PM. There were 5 children (infant- 4 years) sleeping with the Provider. The assistant arrived later with the Provider's child for a total of 14 points. The Provider has files for the children and finger prints have been conducted and are both eligible. No one is currently on medication but the Provider is MAT certified and First Aid/CPR are current for staff. Please call with questions 703-479-4679
Standard #: 22VAC40-111-730-D Description: Based on observation, the household medication was not kept in a locked place. Evidence: 1. Children's Advil was observed on the kitchen counter and Provider said it was for her own child. Plan of Correction: Removed during the inspection.
Standard #: 22VAC40-111-210-A Description: Based on record review ad interview, assistant #1 did not obtain 16 hours of training annually. Evidence: 1. Assistant has attended only 2 hours of annual training in the last year. Plan of Correction: I will have the assistant attend more trainings for the next year.
