An unannounced monitoring inspection was completed on 09/17/2018. There were 12 children, ages 10 months-5 years, totaling 31 points in the care of the provider and an assistant during today's inspection. The children were observed in the following activities: napping, coloring/quiet activities while others slept, having a snack and in free choice play. The infants were observed being allowed to sleep on demand and being fed on demand. The inspector reviewed the emergency supplies and the areas used by the children. The inspector discussed background checks and the provider hiring a new assistant and promoting the current assistant to a substitute provider as needed. Description: Based on observations and discussion, the provider failed to ensure that when meals or snacks are provided by the family day home, the menu for the current one-week period shall be posted in an area accessible to parents. Evidence: The posted menu was dated for September 10-14.

Plan of Correction: The provider will post the current menu.

Description: Based on record review and discussion, the provider failed to ensure that all children's records contain all components as required by the standards. Evidence: The record for Child #3 did not contain the names of persons other than the custodial parents who are authorized to pick up the child and proof of the child's age and identity.

Plan of Correction: The provider will contact the parent and request the information.
