Comments:
An announced initial inspection was conducted from 12:10 p.m. until 2:20 p.m. The Applicant intends to operate a licensed family day home caring for up to 12 children between the ages of 1 month to 12 years of age. The provider is currently caring for children as a voluntarily registered provider. Areas and equipment to be utilized by children in the licensed program were inspected. Required postings, emergency plans and emergency supplies were reviewed. The entire home, to include areas where children will be cared for, bathroom facilities, kitchen and outdoor play areas were inspected. Interactions with the children to include story time, rest period and infant feeding were observed. The applicant states that transportation will not be provided to children in care. Children do not swim or wade in water while in care. There is no medication being administered to children in care at this time. However, the provider does have current Medication Administration Training, should medication be needed. Four children's records and 2 Provider / Assistant records were reviewed. The inspector reviewed General Procedures regarding issuance of a six month conditional license and recommendations at the conclusion of the conditional license. Corrections to violations cited will be reviewed at the first inspection within 2 months of the date of licensure. Please contact your licensing inspector at 757-802-5281 if you have questions.

Standard #: 22VAC40-111-60-B
Description: Based on a review of records, the provider did not ensure that all required components of the children's records were present. Evidence: Four (all) enrolled children's records were reviewed. The record for child #4, who enrolled in the family day home on 9/10/18, did not contain: 1. The name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. The father was listed as the emergency contact on the enrollment form. A second contact was not listed. 2. The record for child #4 did not contain the name and policy number of the child's medical insurance. The provider name was written on the enrollment form but the policy number had not been completed.

Plan of Correction: The provider will ask the parent for this information today at pick up and will have the missing information documented in child #4's record by Monday, October 1, 2018.
