A Subsidy Health and Safety inspection was conducted on 10/15/2018 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from approximately 9:30am-12pm. There were 2 children (8 points) in the direct care and supervision of 1 caregiver. A sample of 2 children?s records, 3 household member's records, 1 caregiver's record were reviewed. The children were observed eating a snack, napping and playing with the provider. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice
Standard #: VENDHOM-000-005-B Description: Based on record review, the provider did not keep a record of daily attendance that documents the arrival and departure of each child. Evidence: The attendance records did not reflect the departure times of the children. Plan of Correction: I will start that documentation today.
Standard #: VENDHOM-000-029-B Description: Based on observation and interview with the provider, the emergency preparedness plan was incomplete. Evidence: The emergency preparedness plan was missing lockdown procedures. The provider stated that she does not have that right now. Plan of Correction: I will complete the plans this week.

Evidence: The evacuation procedures were missing a plan to reunite parents with children after emergency. Provider stated that she does not have that right now.
