54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted today from approximately 10am-12:20pm. There were 2 children (8 points) in the direct care and supervision of 1 caregiver. A sample of 2 children?s records, 3 household member's records, 1 caregiver's record were reviewed. The children were observed eating a snack, napping and playing with the provider. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-111-780-B Description: Based on record review, the provider did not conduct an annual review of the children's records. Evidence: Child A's file was last reviewed 09/11/2017. Plan of Correction: Parents will update file today.
Standard #: 22VAC40-111-50-B Description: Based on record review and interview with provider, the provider's records were not maintained. Evidence: Previous attendance records were not maintained, the provider stated that she could find some previous attendance records. Plan of Correction: I will print more and keep them all.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were incomplete. Evidence: 1 of 2 records reviewed were incomplete. 1) Child A was missing the father's work phone number, the physicians phone number, medication administration form and the medical insurance policy number. Plan of Correction: Parents will update file today.
Standard #: 22VAC40-111-210-A Description: Based on record review, provider did not complete at least 16 hours of annual training. Evidence: The provider could not find proof that 16 hours of training were completed for 2017. Plan of Correction: I never miss training. I will locate the certificates. My required 16 hours of training for 2018 is complete.
