Technical Assistance:
During the inspection the licensing representative discussed with the provider: - caregiver training - caregiver tuberculosis screenings needed by 10/28/18 and 11/1/18 - children's records

Comments:
An unannounced monitoring inspection was conducted today from 11:15am-1:15pm. A total of 12 children [40pts] were in the care of 3 caregivers. Children were observed transitioning from lunch to handwashing and during naptime. A sample of 5 children's records, 4 caregiver/household member records, emergency drills, outdoor play area and other documents were reviewed. Smooth transitions and nurturing interactions were observed between children and caregivers. Areas of noncompliance are documented on the violation notice. Standard #: 22VAC40-111-830-C
Description: Based on a review of records, documentation of emergency drills were not maintained for one year. Evidence: Documentation of shelter-in-place drills was not available at the time of the inspection.

Plan of Correction: I will maintain emergency drill records.

Standard #: 22VAC40-111-960-E
Description: Based on observation, 1 bottle was not properly labeled when more than one infant was in care. Evidence: At the time of the inspection 4 infants were in care. 1 bottle was observed in the refrigerator with a label with a child's name and the date.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-111-90-A
Description: Based on records reviewed, 1 out of 5 children's records did not include documentation that the child has been adequately immunized prior to the child's first date of attendance. Evidence: The record for Child #1 [date of enrollment: 10/1/2018] was missing documentation of immunizations.

Plan of Correction: I will request the record from the parent.

Standard #: 22VAC40-111-240-A
Description: Based on observation, a area outside the family day home was not maintained in safe condition. Evidence: 1 wooden plank on a sandbox cover in the outdoor play area was loose and warped.

Plan of Correction: We will fix it today.

Standard #: 22VAC40-111-320-B
Description: Based on observation, child-resistant protective covers were not installed on all unused electrical outlets. Evidence: A protective cover was not installed on 1 unused outlet in an area that was occupied by children in care during the inspection.
