22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced renewal inspection was completed today. The physical plant, 5 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 2-3:10pm. There were 10 children, totaling 34 points, under the supervision of 2 caregivers. See Violation Notice. Today, the children watched an educational video, ate afternoon snack, enjoyed free play and rested during naptime. The interactions between the children and caregivers were seen to be positive and constructive in nature. The children were served broccoli and potato soup, rice, apple and milk for lunch. The Provider is not certified in MAT (medication administration training) and there are no children with any medication needs. Standard #: 22VAC40-111-100-A Description: Based on file review, 2 out of 5 children's records do not have physical examinations. Evidence 1. Children #2 and #3 do not have physicals on file. Plan of Correction: They are going to the dr tomorrow. I will ask the parents to bring this in on Monday.
Standard #: 22VAC40-111-570-A Description: Based on file review and observations, the Provider did not ensure that caregivers only care for children totaling up to 16 points by using the point system based on their age. Evidence 1. At approximately 2:00pm, there were 10 children in care totaling 34 points. There were only 2 caregivers present during the inspection. Plan of Correction: My assistant left early today for a doctor's appointment.
Standard #: 22VAC40-191-40-D-1-a Description: Based on file review, 1 out of 4 household member/caregivers does not have a completed fingerprint check (FPC). Evidence 1. Caregiver/household member #4 does not have a FPC on file. Plan of Correction: This will be completed.
