An unannounced monitoring inspection was conducted on October 24, 2018, from approximately 11:00 AM to 1:00 PM. Three staff were present with 11 children (25 points). Children were observed in circle time and cleaning up . Five children records, three caregiver record and two household records reviewed on this date. Exit interview conducted with provider.

Standard #: 22VAC40-111-900-A
Description: Based on observation , the provider did not ensure food that is round and small such as uncut grapes not be served to children under four years of age unless the food was prepared before being served in a manner that will reduce the risk of choking. Evidence: During the inspection conducted on October 24, 2018, the children under the age of four were given whole grapes to eat.

Plan of Correction: The grapes will be cut and will be effective immediately.

Standard #: 22VAC40-111-900-B
Description: Based on observation , the licensee did not ensure children were not allowed to eat while walking . Evidence : During the inspection conducted on October 24, 2018, child #3 was walking around eating grapes there cut in small pieces.

Plan of Correction: The children will be put in high chair when eating.

Standard #: 22VAC40-111-180-A
Description: Based on record review the provider did not ensure there was a current Report of Tuberculosis Screening form, every two years from the date of the last screening. Evidence : During the inspection conducted on October 24, 2018, staff #2 and staff #3 most recent tuberculosis test was 09/19/2016.

Plan of Correction: The staff will have there TB test as soon as possible.
