Technical Assistance:
Consultation was given on the annual training requirement (16 hours) per caregiver prior to license expiration of November 23, 2018. Reviewed contract and permission for regular trips and would remove the checked box that the provider transports in her car as she states that she only walks to the park.

An unannounced renewal study was conducted today for the provider from 10:45-12:20. Seven children were present for twenty points with three caregivers which is within ratios for supervision. Children were engaged in clay and coloring, singing, free play, transition to hand washing and lunch. Lunch served today was meat and noddle soup with vegetables, mango, crackers and milk. Emergency medication for one child is present with the provider having current Medication Administration Training (MAT) certification. Reviewed all household member and caregiver background checks, trainings as well as emergency preparation, physical space, cribs and a child's record. The provider has submitted a request from the County to increase capacity and will modify the license once the hearing and paperwork has been submitted. Description: Based on record review, household member (HHM) does not have a current Tuberculosis screening within 2 years of the previous screening or xray. Evidence: HHM #1's TB documentation is over 2 years old as it was done 1/6/2016.

Plan of Correction: Provider will request the HHM complete a new TB screening by the weekend.
