Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and training. A total of 16 hours of training in a child related field will need to be completed by each caregiver during each annual training year.

Comments:
The following inspection took place between the hours of 1 pm through 3:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Sterling, Virginia. At the start of this unannounced monitoring inspection there were a total of 9 children (2 resident children were included) (22 points) with 2 caregivers. A total of 7 children?s files, 2 staff files, and 5 household member?s files were reviewed. The children were engaged in the following: nap time, music, movie time, drop-off from school (school-agers), diapering, bathroom breaks, hand washing, snack time, and other organized activities. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on review, 1 of 7 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (11/07/2018), the documentation of a physical examination for Child #5 was not available for review. 2. The start date for Child #5 was listed as 06/18/2018.

Plan of Correction: Documentation of a physical examination for Child #5 shall be obtained and placed on file.

Standard #: 22VAC40-111-60-B
Description: Based on review, 1 of 7 children's files reviewed indicated that not all of the required information/documentation was contained within each child's record. Evidence - 1. On the date of inspection (11/07/2018), the following documentation was unavailable for review for Child #1: 2 emergency contacts (1 of 2 was available), proof of identification of Child #1, and emergency medical authorization signed by the Parent of Child #1. 2. The start date for Child #1 was listed as 08/20/2018.

Plan of Correction: A completed children's record (model form) will be obtained from the Parent of Child #1 and placed on file.

Standard #: 22VAC40-111-90-A
Description: Based on review, 1 of 7 children's files reviewed indicated that the Provider did not obtain documentation that the child had been adequately immunized (according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations) prior to their start date. Evidence - 1. On the date of inspection (11/07/2018), immunization documentation for Child #1 was not available for review. 2. The start date for Child #1 was listed as 08/20/2018.

Plan of Correction: Immunization documentation shall be obtained by the Parents of Child #1 and placed on file.

Standard #: 22VAC40-111-180-A
Description: Based on review, 1 of 2 staff member's files, and 2 of 5 household member's files reviewed indicated that the Provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening (TB) form (in accordance with the requirements in 22 VAC 40-111-170), every (2) two years from the date of the most current TB documentation on file. Evidence - 1. On the date of inspection (11/07/2018), the following staff did not have current TB documentation on file: Staff #1 (documentation on file was dated - 03/17/2016. 2. The following household members did not have current TB documentation on file: Member #1 (documentation on file was dated - 04/25/2016, and Member #2 (documentation on file was also dated - 04/25/2016.

Plan of Correction: All outdated TB documentation shall be updated.
