Comments:
An unannounced renewal study was conducted today from 1:30pm-3:45pm. A total of 4 children [11points] were present with 1 caregiver. Children were observed during diapering, snack, infant bottle feeding, and transitions from naptime. A sample of 1 caregiver/household member records, 4 children's records, emergency drills, emergency supplies and other documents were reviewed. Violations:
Standard #: 22VAC40-111-770
Description: Based on observations, the battery operated radio was not operable during the inspection. Evidence: The battery operated radio was not working at the time of the inspection.

Plan of Correction: I will purchase a new one.

Standard #: 22VAC40-111-830-C
Description: Based on records reviewed and interview, documentation of emergency evacuation drills were not maintained. Evidence: Provider stated that an emergency drill was practiced on 10/22/2018; however, the drill was not documented on the emergency drill log.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-100-A
Description: Based on records reviewed, 2 out of 4 children's records did not include documentation of a physical exam within 30 days of enrollment. Evidence: The records for Child #2 and #4 [both enrolled on 6/28/2018] were missing documentation of a physical.

Plan of Correction: The parent has appointment scheduled for the children.

Standard #: 22VAC40-111-60-A
Description: Based on records reviewed, 2 out of 4 children's records did not include the required information. Evidence: The records for Child #2 and #4 were missing the name, address and phone number for 1 of 2 required emergency contacts.

Plan of Correction: I will have the parent add this information to the record.

Standard #: 22VAC40-111-320-B
Description: Based on observations, protective covers were not installed on all unused electrical outlets. Evidence: During the inspection, 3 outlets were missing protective covers in rooms occupied by children.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-550
Description: Based on observation, a play pen was used as a designated sleeping space. Evidence: Upon inspector arrival, Child #1 was observed asleep in a play pen for approximately 3 minutes.

Plan of Correction: I will make sure to place the child in a crib until they turn 12 months old in a couple weeks and begin to transition to a rest mat.
