An unannounced renewal inspection was completed on 11/15/2018. Present during the inspection were 2 children, both of which were 2 years old, totaling 4 points, in the care of one provider. The inspector reviewed 5 children's records and one provider record during today's inspection. The provider stated that there was no medication being administered at this time. The children were observed in the following activities: free choice activities, music and movement, having a morning snack and participating in learning activities; the daily scheduled was altered due to a 2 hour delay because of inclement weather. The inspector reviewed the emergency preparedness plans, the required information for parents and emergency supplies during the inspection today. The inspector discussed the following with the provider: an upcoming field trip, continuation of trainings by an outside source and the changes to the subsidy requirements. Standard #: 22VAC40-111-60-B Description: Based on observations, record review and discussion, the provider failed to ensure that each child's record shall contain all of the elements as required by the standards. Evidence: The record for Child #2 did not contain the following information: policy number of the child's medical insurance and results of the health examination for the child as required by 22VAC40-111-100. Plan of Correction: The provider contacted the parent and requested the information during the inspection.
