Comments:
Thank you for your assistance during this announced initial inspection. This on-site visit was conducted on November 19, 2018 from 1:10 p.m. through 2:00 p.m. No children were/are in care at this time. The records for the provider and two household members were reviewed. Areas reviewed today were as follows: Indoor and outdoor areas of the home, emergency supplies, posted information, equipment used for the children in care, and supplies. There are a wide variety of materials for the children in care. A recommendation will be made for issuance of a six (6) month conditional family day home license.
