Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing to include new assistant, and a review of children, staff, and household files. Annual training of 16 hours shall be reviewed at the next unannounced inspection.

Comments:
The following inspection took place between the hours of 11 am through 2 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located near Aldie, Virginia. At the start of this unannounced renewal inspection there were a total of 6 children (19 points) with 2 caregivers. A total of 8 children?s files, 2 staff files, and 4 household member?s files were reviewed. The children were engaged in the following: infant feeding, diapering, hand washing, toy play, story time, learning activities, music, song, lunch, nap time, and other organized activities. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on review, 1 of 8 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) prior to the child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (11/14/2018), documentation of a physical examination for Child #6 was not available for review. 2. The start date for Child #6 was listed as 09/05/2018. 3. Child #6 was observed in care on the date of inspection.

Plan of Correction: Documentation of a physical examination shall be obtained for Child #6 and placed on file.

Standard #: 22VAC40-111-60-B
Description: Based on review, 1 of 8 children's files reviewed indicated that not all of the required information was contained within each child's record. Evidence - 1. On the date of inspection (11/14/2018), the addresses for both emergency contacts for Child #6 were not available for review. 2. The insurance policy number for Child #6 was not available for review.

Plan of Correction: All missing information shall be obtain from the child's parent and placed on file.

Standard #: 22VAC40-111-90-A
Description: Based on review, 2 of 8 children's files reviewed indicated that the Provider did not obtain documentation that all children had been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations prior to attending the Family Day Home (FDH). Evidence - 1. On the date of inspection (11/14/2018), immunization pages were not available for review for: Child #6, and Child #7. 2. The start date for Child #6 was listed as 09/05/2018. The start date for Child #7 was listed as 10/16/2018.

Plan of Correction: All missing documentation shall be obtained for each child and placed on file.

Standard #: 22VAC40-111-90-C
Description: Based on review, 2 of 8 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for children (who were not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (11/14/2018), the following children had immunization pages that were older than 6 months: Child #4, and Child #5. 2. The immunization pages for Child #4 and Child #5 were both dated - 04/23/2018.

Plan of Correction: Updated immunization pages shall be obtained for both children and placed on file.
