Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:15 am - 11:45am. There were eleven children in care for a total of 30 points with two caregivers. The children were observed playing, watching cartoons, during snack time, and listening to a story. Two caregiver's records, one household member's record and five children's records were reviewed. One violation from the previous inspection had been corrected. Areas of non-compliance are identified on the violation notice. Violations:
Standard #: 22VAC40-111-150-D-1
Description: Based on record reviews, the family day home failed to have a caregiver present with current certification in cardiopulmonary resuscitation (CPR). Evidence: 1. The inspector reviewed two caregiver's records. Caregiver's records #1 and #2 contained CPR certification that expired 11/21/18.

Plan of Correction: The provider will call the trainer today and schedule CPR training for both caregivers. The provider will send copies of the new certificates to the licensing office when they are obtained.

Standard #: 22VAC40-111-150-D-2
Description: Based on record reviews, the family day home failed to have a caregiver present with current certification in first aid. Evidence: 1. The inspector reviewed two caregiver's records. Caregiver's records #1 and #2 contained first aid certification that expired 11/21/18.

Plan of Correction: The provider will contact the trainer today and schedule first aid training for both caregivers. The provider will send copies of the new certificates to the licensing office when they are obtained.

Standard #: 22VAC40-111-180-A
Description: Based on record reviews, the family day home failed to obtain repeat tuberculosis (TB) screenings for caregivers. Evidence: 1. The inspector reviewed two caregiver's records. Caregiver's records #1 and #2 contained TB screenings that were over two years old. Both screenings were dated 11/22/16.

Plan of Correction: The provider will contact the health department today and schedule the repeat TB screenings for both caregivers.
