Comments:
The licensing inspector conducted an unannounced renewal inspection on Monday, November 26, 2018 from 11:20am to 12:46pm. The children were observed playing with toys and working on small group lessons. They were later observed eating lunch and resting on mats during rest time. Today, there were eleven children, totaling 24 points, present with four caregivers. The provider does not administer medication. The provider does provide transportation. The provider does have a pet. The dog's rabies vaccination is current. Five children?s records, four caregivers? records and one household record were reviewed during the inspection. The violations from the previous inspections were checked for corrections. There was one repeat violation found during today?s inspection. If you have any questions about this inspection, please contact the licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on a review of records on 11/26/2018, the provider did not ensure that one of the five records reviewed contained the required information. EVIDENCE: The record of Child #5 did not contain: 1. The name and phone number for the father's place of employment. 2. The phone number and address for the place of employment listed for the child's mother. 3. The address and phone number for the child's physician 4. The address and phone number for the two emergency contacts listed

Description: Based on a review of children's records on 11/26/2018, the provider did not ensure that before a child may attend the family day home, they obtained documentation that the child has been adequately immunized. EVIDENCE: The record of Child #5, who started at the family day home on 11/06/2018, did not contain documentation of immunizations.
