Technical Assistance:
Discussed separating toothbrushes grouped together in the cup located in the downstairs bathroom. Separating the toothbrushes will prevent the spread of germs.

An unannounced inspection was conducted with another agency in response to a complaint received in our office on 11/8/2018. 8 children (19pts) were present with the Provider, 2 household members and 1 assistant. Regarding the space/amount of rooms used for child care, the complaint is valid. Other violations cited during the inspection are available for review on the Violation Notice. Complaint related: No
Description: Based on observation, bottles were not refrigerated and labeled with the child's full name and the date, with more than one infant in care. Evidence: 2 bottles were observed on the counter, without labels. 2 infants were in care.

Plan of Correction: They have been labeled as of 11/19/2018.

Description: Based on observation and interviews, the provider did not ensure compliance with the standards and the terms of the current license issued by the department. Evidence: The latest renewal application, dated October 2017, stated 3 rooms were used for child care. 5 children were observed in the 3 rooms of the basement, 2 children were on the main level and 1 child was in an upstairs bedroom.

Plan of Correction: 2 children in the main level were having breakfast as they came late (to prevent disturbing the schedule). 1 child placed upstairs due to the request of the parents for that child is not sleeping well.

Description: Based on interview and record review, records and reports for caregivers required by this chapter were not maintained and made accessible to the department's representative for two years from the date of termination of services for a child, date of separation from employment for caregivers, or date of termination of residence for a household member, or unless specified otherwise. Evidence: Records were not available for an employee that was employed less than one year ago.

Description: Based on interview and observation, the Provider did not maintain an up-to-date record at the family day home for each enrolled child. Evidence: The record for Child #2 was not available for review.

Description: Based on observation, hazardous materials were not stored in areas inaccessible to children. Evidence: Cleaning agents were located under the kitchen sink, with no lock. The caregiver stated children sometimes eat in the kitchen so the area is accessible to children.

Plan of Correction: Safety locks were installed immediately on 11/19/2018.

Description: Based on observation, child-resistant protective covers were not installed on all unused electrical outlets accessible to children under five years of age. Evidence: Protective covers were missing from outlets on the lower level and main level. Both floors of the home are accessible to children under five years of age.

Plan of Correction: There were one receptacle downstairs and one on the main floor using prior to the visit. Protective covers have reinstalled after use.

Description: Based on observation, equipment and materials used by a child were not appropriate to the age, size, ability and interest of the child. Evidence: Child #1 was observed asleep in a swaddling sack, face down on a mat, unable to use his hands to push up if needed. The age range for the sack is 3-6 months and the child is 7 months old.

Plan of Correction: Child #1 was swaddled with the consent of the parent, and kept face down due to obstruction of airway by phlegm. Please see attached letter of consent.

Description: Based on observation, a child was not provided with an individual crib for resting or napping. Evidence: Child #1 was observed sleeping on a mat, on the floor, in an upstairs bedroom.

Plan of Correction: Mat was provided to prevent suffocation and as a temporary solution for the parents request.
