Technical Assistance:
Discussed with the provider: assuring that bottles with either breast milk or formula is labeled with the child's name and the date.

Comments:
An unannounced monitoring inspection was conducted beginning at 9:30 and ending at 10:30. There were 11 children in care with the provider and one assistant. The children were ages 4 months through 5 years . The older children were observed participating in table activities and the younger children were playing with age appropriate playthings on the floor and having circle time. Reviewed the files for 11 children and one new assistant. - see violation notice. Emergency escape drills and shelter in place drills are up to date. Standard #: 22VAC40-111-100-A
Description: Based on a review of 11 children's records, it was determined that 1 child, did not have a physical examination. Evidence: Child A, who has been in care sine 6/25/18 does not have a physical (well baby check) on file

Plan of Correction: The provider stated that the parent said that the pediatrician does not do a physical until 12 months. The provider will tell the parent that she needs to notify the physician that it is a requirement for any age child who is in childcare.

Standard #: 22VAC40-111-200-A
Description: Based on a review of the staff record for the Assistant, it was determined that the Assistant has notr been given Orientation Training. Evidence: Staff #1, who began work on 8/20/18 has not had orientation training documented.

Plan of Correction: The provider stated that she will get the form from the web site and complete it with the assistant.
