- Technical Assistance:
Technical assistance today included the diapering area, record keeping and staff- facilitated active play opportunities for children.
An unannounced renewal inspection was conducted today from 10:35 am - 1:30 pm. There were seven children in care for 20 points with the provider and assistant. I observed children at free play with a wide variety of toys, preparation for lunch and the beginning of naptime. If you have any questions, please contact me at firstname.lastname@example.org.
Standard #: 22VAC40-111-800-A Description: Based on a review of documents today, there was no written emergency preparedness and response plan available for review. Evidence: Although parents signed the Procedures for Emergency form, the more detailed required document that covers evacuations, relocations and shelter in place was not available today. Plan of Correction: I will fill it out today and hang it up.
Standard #: 22VAC40-111-960-E Description: Based on observation during today's inspection, not all infant bottles were labeled. Evidence Although the provider has a system for designating bottles with a round colored sticker, it did not contain names as required. Plan of Correction: Corrected during inspection.
Standard #: 22VAC40-111-100-A Description: Based on a review of children's records today, not all children had documentation of physical exams as required. Evidence: There were no available physical exams for Child # 1, who began 5/16, and Child # 2, who began 6/17. Plan of Correction: I will talk to the parents. One parent brought it in but it is not available today.
Standard #: 22VAC40-111-120-B Description: Based on a review of record keeping today, some assistant provider's required records were not available. Evidence: There was only on reference documented for Assistant # 1 who began 11/18. Plan of Correction: I will get another reference today.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records today, some did not have all documentation as required. Evidence: Child # 2, 4 and 5 did not have documentation of annual review by parents of emergency contact information. Plan of Correction: I will talk to the parents.
Standard #: 22VAC40-111-90-A Description: Based on a review of records today, one child in care did not have an immunization record on file before beginning care. Evidence: Child # 3 began care 10/18 and did not have a shot record available today. Plan of Correction: I will send a text to the parent today.
Standard #: 22VAC40-111-180-A Description: Based on a review of household records today, one adult household member did not have a current tuberculosis screening documented as required. Evidence: Household member # 1 had a TB screening that expired 9/18. Plan of Correction: He will go to get the screening.
Standard #: 22VAC40-111-530-B Description: Based on observation during today's inspection, some cribs in use by infants did not have tight fitting bottoms as required. Plan of Correction: I will buy new sheets so they are tighter.
Standard #: 22VAC40-111-530-F Description: Based on observation, an infant was observed sleeping in a crib with soft bedding under his head. Evidence: An infant (age 5 months) was observed to be sleeping with a rolled up blanket under his head. The provider reports that this was per the parent's request. Plan of Correction: The blanket was removed. It was requested by the mother but I will explain to her that we need a doctor's note.
Standard #: 22VAC40-191-40-D-1-b Description: Based on a review of records today, there were no results of one of the background checks as required. Evidence: Although the Search for the Central Registry was mailed in November, there are no results as required for Assistant #1 before beginning work with the children. Plan of Correction: I am checking with the Office for Background Investigations now.
