Comments:
An unannounced monitoring visit was conducted today between the hours of 9:30am through 11:00am with the provider on site. There were 2 children (4 points) with the provider. A sample of 4 children's files were reviewed. The children were engaged in the following: toy play, puzzles, reading books with the provider, playing with play dough and singing. . Interactions between the children and provider was positive. The physical space, emergency supplies and attendance records were reviewed. The home was clean, organized and contained an abundant supply of developmentally appropriate materials. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-890
Description: Based on observation, when food is brought from home, the food container was not clearly labeled. Evidence: 1. One zip lock bag with Child #4's snack was in the providers refrigerator, but was not labeled with the child's name on it.

Plan of Correction: I will make sure the food brought from home is labeled with the child's name and date.

Standard #: 22VAC40-111-920
Description: Based on observation and interview, the menu for the current one week period was not given to parents or posted or placed in an area accessible to parents. Evidence: 1. The provider stated she provides snacks or light meals for the children, but did not have a menu this week.

Plan of Correction: I will make a menu tonight.

Standard #: 22VAC40-111-60-B
Description: Based on review of 4 children's records, the records were not complete. Evidence: 1. Child #1's file (start date 11/22/2018) was missing one emergency contact name, address and phone number. 2. Child #2's file (start date 12/07/2018) was missing the medical insurance policy number. 3. Child #3's file (start date 12/06/2018) was missing one emergency contact phone number and the 2nd emergency contact name, address and phone number, and the insurance policy number. 4. Child #4's file (start date 12/10/2018) was missing all required information, except the child's physical, immunization record and doctor's office information.

Plan of Correction: I will ask the parent's for the information today.

Standard #: 22VAC40-111-70-B
Description: Based on interview and record review, the provider did not obtain the parent's written acknowledgement of the receipt of the information required to be given by the first day of attendance. Evidence: 1. Child #4's began with the provider on 12/10/2018 and did not have the parent's written acknowledgement of the receipt of the Information for parents form, policy for administration of medication form, provisions of the emergency plan or the liability insurance declaration form.

Plan of Correction: I will get the forms back from the parent.
