An unannounced monitoring inspection was conducted today between the hours of 8:05 am through 9:20. There were a total of 7 children (17 points) with 2 caregivers. A total of 3 children's files and 5 staff/household member's files were reviewed. The children were engaged in the following: breakfast, hand washing, and toy play. The Provider has a new puppy, too young for rabies shot. The record will be reviewed at the next inspection. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-60-B Description: Based on record review, each child's record is not complete. Evidence: Child #1 record is missing the medical insurance policy number. Plan of Correction: I will get the information from the parent.
Standard #: 22VAC40-111-180-A Description: Based on record review, caregiver has not updated their TB screening every two years from the date of the first screening. Evidence: Caregiver #1 does not have a current TB screening on file. TB screening dated 2/23/16. Plan of Correction: I will get a copy of the TB.
