Technical Assistance:
Provided technical assistance with the provider regarding television. As a reminder: Use of media such as, but not limited to television, videos, video games, and computers shall be limited to not more than a total of two hours per day and the media must be produced for or suitable for children.

An unannounced monitoring inspection was conducted from 10:00 am - 11:15 am. There were 9 children being supervised by the provider and one assistant ages, 7 months, 10 months, 12 months, 12 months, 12 months, 12 months, 2 years, 2 years, and 2 years for a total of 30 points. The children were observed participating in structured center play, crafts, dancing, and preparing for lunch. Lunch is provided from home. Nine children's records were reviewed and in compliance. The provider and one assistant records were reviewed and in compliance. The most recent emergency escape drill was conducted on 11/30/2018 and the most recent shelter in place drill was conducted on 11/29/2018. There are reportedly no medications being administered to children in the family day home at this time. The provider is current in CPR and First Aid Training. Standard #: 22VAC40-111-960-E
Description: Based on observation of infant bottles it was determined that not all bottles were labeled with the child's full name and date. Evidence: Two bottles were observed in the refrigerator that were not labeled with full name and date. These bottles reportedly belong to Child A and Child B.

Plan of Correction: The bottles were labeled during the inspection

Standard #: 22VAC40-111-280
Description: Based on observation of the bathroom used by the children it was determined that not all potentially poisonous substances such as cleaning agents were inaccessible to children. Evidence: Toilet cleaner was observed in the unlocked bottom cabinet.

Plan of Correction: The cleaning agent was removed and locked during the inspection.

Standard #: 22VAC40-111-320-B
Description: Based on observation of the nursery it was determined that not all unused outlets were covered with child-resistant protective covers. Evidence: An electrical outlet located behind a crib accessible through the crib slats did not have child-resistant protective covers installed.

Plan of Correction: Child-resistant protective covers were placed in the electrical outlet during the inspection.

Standard #: 22VAC40-111-340-C
Description: Based on observation of the bathroom it was determined that not all required components were observed in the bathroom. Evidence: There was no toilet tissue accessible to children in the bathroom.

Plan of Correction: Toilet tissue was replenished in the bathroom during the inspection.
