The licensing inspector conducted an unannounced monitoring inspection, at which play groups, personal hygiene, lunch time, activities, and rest time were observed. Children were observed; and staff and children files and other documentation were reviewed. Eleven (11) children, aged 3 months-3 years totaling 28 points, were found to be in care with the provider and assistant. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 11:40am and concluded at 1pm.

Standard #: 22VAC40-111-920
Description: Based on observation the provider failed to maintain compliance with this standard as required relating to posting of the menu. Evidence: The posted menu was for the previous week, not the current week as required.

Plan of Correction: The menu is made, but had not yet been posted on the parent board. This will be posted immediately. Menus will be posted each Monday morning to ensure future compliance.

Description: Based on review of children records the provider failed to maintain compliance with this standard as required relating to emergency contacts. Evidence: Child record #5, enrollment date 10/21/18, had only one of the required two emergency contacts listed.

Plan of Correction: I will obtain this information from the parent immediately. I will continue to ensure files are compliant, and update any changes as needed.
