A Subsidy Health and Safety inspection was conducted on October 31, 2018 to review supplemental health and safety requirements. There were 3 children in care. Based on the children's ages, the points totaled 6. The children were observed as they participated in varied activities which included: coloring, free play, hand washing, lunch and nap time. A sample of 5 children's records was reviewed. The Provider's record was also reviewed. As reported, medications are not administered by the Provider. Documentation of emergency drills was reviewed. As reported by the Provider, an assistant or substitute provider are not being scheduled currently. When an assistant is hired, the Provider will establish a staff record prior to the individual working with the children. A licensing inspection was also conducted this date, the two combined inspections cover all subsidy health and safety requirements. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. The on-site inspection was conducted from approximately 11:00AM to 1:45PM. Findings were reviewed with the Provider at exit. Description: Based on staff interview and reviewed documentation, the Provider failed to practice lockdown procedures at least annually. Evidence: Documentation of emergency drills was reviewed by the LI, there was no documentation for a practiced lockdown drill.

Plan of Correction: The Provider will practice a lock down drill and will keep documentation.
