Comments:
An unannounced renewal inspection was conducted on 01/04/2019 from 11:40am - 12:50pm. At the time of entrance there were four children in care with the provider. The ages of the children counted for a total of ten points. The late morning routine of the home was observed which included lunch and nap. Records were reviewed for the four children in care, the provider and one household member. The entire home and backyard play area was additionally reviewed. The violations are listed on the violation notice and were reviewed with the provider during the inspection.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review and interview, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. There was not a documented physical on file for child 1 and child 2 who were in care during the inspection. a. Both children have been in care more than 30 days. 2. The provider verified that a physical was not available for child 1 and child 2.

Plan of Correction: The parent's of child 1 and child 2 were asked to provide a documented physical within 30 days.

Standard #: 22VAC40-111-60-B
Description: Based on record review and interview, the provider failed to ensure that children's records contain all of the required elements. Evidence: 1. The enrollment record for child 2 lacked the following required information: a. Name, office address and telephone number of the child's physician; b. The street address of the first emergency contact. 2. The provider verified that this information was not on file for child 2.

Plan of Correction: This information will be added to the record of child 2 today.
