An unannounced renewal inspection was conducted on 1/11/19 from 11:00am until 12:00pm. At the time of entrance, there were 4 children in care, for a total of 11 points, with the provider. A sample of 4 children's records and 1 provider record were reviewed. Children were observed watching an educational program on television, eating lunch and resting quietly during nap time. Hand washing, lunch service and infant diapering was also observed. First aid and emergency supplies, the emergency preparedness plan and family day home postings were reviewed. The provider stated there have been no accidents or injuries to children that require first aid or medical care since the previous inspection. Two violations in the areas of physical environment and equipment and emergencies are identified on the violation notice and were discussed with the provider during the exit interview.

Standard #: 22VAC40-111-830-C
Description: Based on record review and interview, it was determined the provider did not maintain documentation of emergency evacuation and shelter-in-place drills. Evidence: 1. The documentation log for emergency evacuation and shelter-in-place drills was not available for review during the inspection. 2. The provider stated the drills had been completed and documented each month; however, the log was accidentally thrown away.

Plan of Correction: The provider stated that going forward, emergency drill logs will be posted on the wall of the family day home in order to avoid losing or accidentally disposing of the logs.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider did not ensure that areas and furnishings of the family day home, inside and outside, shall be maintained in a safe and operable condition. Unsafe conditions shall include, but not be limited to, protruding nails, bolts, or other components that could entangle or could snag skin. Evidence: The fence in the backyard had 2 protruding nails at the bottom and were accessible to children in care. The screws were rusty and had sharp points.

Plan of Correction: The provider stated the screws would be removed from the fence today.

A compliance history is in no way a rating for a facility.

The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.

Virginia Quality is a voluntary quality rating and improvement system for early care and education facilities serving children ages birth through pre-K. Eligible child care facilities must be fully licensed, licensed exempt and a VDSS subsidy vendor, or a voluntary registered day home and a VDSS subsidy vendor. Only programs enrolled in Virginia Quality will display a rating. Virginia Quality contact information for your region is available at the following link Regional Contacts.
